Step 1: Registration Documents
Completing registration documents will give you an opportunity to review our office policies to determine if we are a good fit for you and your child. Also, we recommend contacting your insurance company to be sure we are in-network.
Step 2: Previous Medical Records
While completing registration documents, you will be given the option to complete a medical record release. This will allow us to obtain your child’s records and immunization history from your previous doctor.
We MUST receive and verify your child’s medical records BEFORE scheduling a well care appointment. Sorry, no exceptions.
After we receive and verify your previous medical records, we will contact you to schedule your first well care appointment. It's important to know that this process may take up to 3 - 20 business days depending on how quickly your previous doctor processes the record request.
