INCLUDES the following services:

- - Growth assessment
- - Most immunizations
- - Dietary, exercise and lifestyle counseling
Chronic conditions, rash, reflux, allergies, fever, asthma, behavioral care, medication refills, ear pain, headache, cough, stomach pain, sore throat, some preventive tests, some screenings and
ALL other services or concerns not pertaining to physical exam, growth assessment and immunizations are
NOT INCLUDED in an annual wellness visit.
